Start with the failing surface and check the authority that owns it.

Aether keeps authority on the server, so a control can disappear or a change can be rejected when the current role, protocol, projection, or document revision does not permit it. Do not treat a stale visible control as proof that an action succeeded.
- Connection and access: sign-in, missing worlds, membership, unavailable links, and controls that do not appear.
- Map and vision: wrong grid, missing tokens, darkness, walls, doors, fog memory, and Player View.
- Content and History: Library visibility, sharing, references, Roll Tables, rejected changes, blocked undo, and permanent operations.
If a failure is not covered, record the page, world, role, visible message, and the action that led to it. A screenshot of a control without those facts is rarely enough to identify the authority path that refused it.
